DOT Physical Exam

 

  • Fill out a 1-page brief health history
     

  • Complete your DOT physical exam with doctor Hawkins, D.C.

    • review of brief medical history

    • height, weight, blood pressure, pulse rate, range of motion, breathing, eyes, ears

    • discuss special medical conditions

    • urinalysis for blood sugar
       

  • We complete your paperwork

 

You receive a laminated DOT medical card for your CDL to keep with you at all times while you are driving and also a copy of the DOT physical form. We will fax or e-mail the DOT physical form to your company.

 

Costs are reasonable, hours are convenient, and we accept major credit cards, cash, and approved checks.
